Revision surgery (“re-do”) patients need to call 346-571-7500 prior to scheduling appointment as we require medical records first.
Surgery patients requesting “second opinion” after surgery ordered by another orthopedic surgeon.
Surgery patients who have exhausted conservative therapy and are ready to proceed with surgery.
Conservative Therapy for joint pain includes: supervised outpatient physical therapy, anti-inflammatory medications, weight loss, and trial of injections (steroid, hyaluronic acid, or PRP).
If you do not meet criteria for Dr. Kreuzer New Patients above, please schedule with another provider below who may begin conservative therapy.

Request for Medical Records
INOV8 Orthopedics follows the current rules of the Texas State Board of Medical Examiners (Texas Medical Board) related to the release of medical records and corresponding charges to patients and other parties. Medical records are $25.00 (minimum). To access your medical records without a fee, use our Patient Portal found above.
To submit a Medical Records Request or X-ray Request: Complete the Authorization for Use/Disclosure of Protected Health Information Form. You may fax or mail the form to the fax number or address at the bottom of the form.

Please include payment of fees (shown on form) with request. Payment may be mailed with request. For other payment options, please call us. Please allow up to 15 days once we receive your request and applicable fee to process your medical records and/or X-rays.
